About Me

Maren Sheese serves Centerstone of Indiana, a community mental health center (CMHC), as the Director for Integrated Health.  She is also the Project Director for a SAMHSA funded Primary and Behavioral Healthcare Integration (PBHCI) grant.  This is the second SAMHSA PBHCI grant she had directed.  She is an adjunct professor at Indiana University, Bloomington, IN. She obtained her Project Management Professional (PMP) in June 2016.  Her work at Centerstone is centered on building and implementing services that will improve the physical health status of adults with serious and persistent mental illness (SPMI). Patient interventions feature a wrap-around care model where health coaches work in tandem with patients and their respective health care providers (internal and external). Service is delivered from a strengths and motivational interviewing perspective and utilizes evidence based protocols.  Integrated programming has led to outstanding patient health outcomes specifically in the areas of: weight management, unmanaged or high blood pressure/cholesterol, nicotine dependence and diabetes. Innovative programming using technology enabled care has led to impressive cost savings relative to hospitalizations and improved health outcomes of patients served.
